TH Back to School Suicide Prevention Survey

**SURVEY IS FOR PARENTS & GUARDIANS ONLY

BACK TO SCHOOL KICKOFF

TH is asking community members to take a moment to complete a survey for Suicide Prevention programming.  This survey is for the “Back-to-School” events taking place throughout the Reservation.  By completing the survey ahead of time, community members can let Behavioral Health staff know at any of the “Back-to-School” events, which will qualify for entry into a drawing.
