Services Provided
Tribal Care Coordination (TCC) is focused on providing care coordination through a range of services including connecting patients to community resources, coordinating and implementing care plans, patient advocacy, and communicating with the patient’s primary care provider.
Care coordination exchanges information between health entities to ensure accurate, up-to-date information is provided to benefit the patient’s care. TCC takes a holistic approach by providing each patient with a care team that includes nurses, social advocates and community health representatives.
