Services Provided
Tribal Health has received grant funding from the Special Diabetes Program for Indians (SDPI) for several years, under the Indian Health Service (IHS), to assist our efforts of treating and preventing diabetes in our community.
- Support & Education: Offers evidenced-based diabetes self-management education and support education in a three-part series to all patients with diabetes as a foundation to empower patients to navigate self-management decisions and activities.
- Glucose Monitoring: Assists patients with getting access to cutting-edge technology for diabetes management including continuous glucose monitoring (CGM) as well as traditional supplies for blood glucose testing including meters, test strips, and lancets.
- Diabetes Evaluation: Evaluations of diabetes medication regimen by a clinical pharmacist with recommendations sent to primary care provider without a referral or full disease state management by the pharmacist with provider referral.
- Financial Support: Helps active diabetes services patients pay for a portion of their dental care, eyeglasses, orthopedic accessories, and aquatic center fees. To be considered active, patients must see a member of the diabetes services care team at least annually.
